چکیده

Background: Metacarpal fractures are among the most common fractures. The majority of metacarpal shaft fractures can be treated conservatively. There are numerous indications for operative treatment and various fixation techniques. The purpose of this prospective study was to assess the surgical and clinical outcomes of metacarpal shaft fractures using intramedullary fixation by K-wire (Kirschner wire). Materials and Methods: 15 cases of Metacarpal shaft fractures underwent surgery with Intra medullary fixation by K-wire. The study was conducted from March 2015 to February 2016 admitted to Navodaya Medical College and research center, Raichur. We evaluated the DASH score to determine the outcomes. Results: All fractures got united in our study and all patients were available for follow up. Two superficial wound (pin site) infections noted which healed with intravenous antibiotic treatment and regular pin site dressing. There were no major complications noted. All patients regained full flexion at the MCP and IP joints but two patients had a mild extensor lag of about 10°. At final follow up the mean DASH score in our study was 4.3 (range: 0-7.2). Conclusion: Closed or minimally invasive fixation with Kirschner wire (k-wire) is a safe method and can be performed with minimal complications. It can be seen as good choice in selective metacarpal shaft fractures.

Surgical Fixation of Fourth and Fifth Metacarpal Shaft Fractures with Flexible Intramedullary Absorbable Rods: Early Clinical Outcomes and Implications.

BACKGROUND To avoid the irritation of tendons and soft tissues as well as hardware-related problems, we designed an intramedullary fixation with bioabsorbable rods for the treatment of the metacarpal shaft fractures. METHODS Five patients with nine shaft fractures of the fourth and fifth metacarpi were treated with intramedullary absorbable implants and followed up with an average of 4.2 mont...

Comparison between Intramedullary Nailing and Percutaneous K-Wire Fixation for Fractures in the Distal Third of the Metacarpal Bone

BACKGROUND To compare clinical and radiographic outcomes between intramedullary nail fixation and percutaneous K-wire fixation for fractures in the distal third portion of the metacarpal bone. METHODS A single-institutional retrospective review identified 41 consecutive cases of metacarpal fractures between September 2009 and August 2013. Each of the cases met the inclusion criteria for close...

RESPONSE OF METACARPAL FRACTURE FIXATION CONSTRUCTS TO PHYSIOLOGICAL LOADINGS

This study compares the functional capabilities of different metacarpal fixation constructs under physiological loadings in an attempt to identify the optimal construct rather than the strongest one. One hundred and twenty-six preserved human metacarpals were mechanically tested after oblique osteotomies and internal fixation. Maximum load to failure, average structural rigidity, and energ...

Intramedullary nailing of metacarpal shaft fractures.

Uncorrected bony deformity or stiffness resulting from a metacarpal shaft fracture can produce a significant functional or cosmetic deficit. Intramedullary fixation of metacarpal shaft fractures using small flexible rods can provide stable internal fixation while minimizing the extent of soft tissue trauma that is associated with more extensive surgical techniques such as plate or screw fixatio...
